To determine the sensitivity to change and minimal clinically important difference (MCID) for the self-administered Locomotor Capabilities Index-5 (LCI-5) in people with lower limb amputation undergoing prosthetic training.
Design
Prospective single-group observational study.
Methods
The LCI-5 was administered to 110 patients (69 males [63%]; median [interquartile range] age, 60 [48–69] years) before and after prosthetic training. The external anchor administered after the program was a 7-point Global Rating of Change Scale (GRCS) designed to quantify the effect (improvement or deterioration) of the intervention.
Results
Test–retest reliability of the LCI-5 (n = 30) was high (intraclass correlation coefficient [ICC2,1] = 0.92). The minimum detectable change at the 95% confidence level was 5.66 points. After triangulating these results with those of the mean-change approach and receiver operating characteristic (ROC) curve analysis (area under the ROC curve ≥ 0.90), based on a different GRCS score splitting, we identified 2 cutoffs for the LCI-5: a change of 7 points, indicating the MCID, and 12 points, indicating “large improvement” in locomotor capabilities (12.5% and 21.4% of the maximum possible score, respectively).
Conclusions
The LCI-5 showed a high ability to detect change over time (responsiveness). The 2 proposed values (MCID of 7 points and large improvement of 12 points), based on a mix of distribution- and anchor-based approaches, represent cutoffs that can accurately identify 2 different levels of true change (as perceived by the patient) in locomotor capability after prosthetic training.Permalink : ./index.php?lvl=notice_display&id=84110

The PLUS-M 12-item Short-Form is a self-questionnaire that assesses the perceived capacity of lower limb amputees (LLAs) to perform a number of daily-life activities. Its psychometric properties are excellent (intraclass correlation coefficient [ICC] > 0.9, fast administration and scoring, normative data available), and it can be used in clinical practice or for research purposes.
Objective
We aimed to develop a French version of this questionnaire and to assess its psychometric properties.
Methods
We followed international recommendations for translation and cross-cultural validation of questionnaires. In total, 52 LLAs (age 53 ± 16, 40 males, 28/12/12 transtibial/Gritti-Stokes/transfemoral, 20/28/4 ischemic/traumatic/other) participated. Criterion and construct validities were assessed with the Pearson correlation coefficient (PCC) between the PLUS-M 12-item Short-Form and other constructs (Prosthetic-Profile-of-the-Amputee-Locomotor Capabilities Index, Activities-specific Balance Confidence scale, 2-min walking test and Timed Up and Go test), internal consistency with the Cronbach α and reliability with the ICC in 46 individuals who completed the questionnaire twice in a 7-day interval.
Results
The mean (SD) PLUS-M 12-item Short-Form T-score was 56.1 (7.8; range 40.3 to 71.4). Construct and criterion validity, internal consistency and reliability ranged from low to excellent (r = 0.43 to 0.84, P < 10−2 to 0.002; Cronbach α = 0.90, ICC = 0.89 [0.81–0.94]). We found no floor or ceiling effect.
Conclusions
The French version of the PLUS-M 12-item Short-Form has good to excellent psychometric properties, comparable to those of the original version. In total, 20 participants with sensorimotor impairments with various etiologies and able to stand for at least 30 sec without human or technical assistance were video-recorded during evaluation with the Mini BESTest. From this video-recording, we calculated inter-rater and intrarater reliability (intraclass correlation coefficient = 0.974–0.988), internal consistency (Cronbach alpha = 0.895–0.929), standard error of measurement (1.05 and 1.63), and minimal detectable change at the 95% confidence interval (2.91 and 4.51). All values were comparable to those previously reported for the original version of the Mini BESTest. Furthermore, no significant ceiling or floor effect was detected. Therefore, the translated and transculturally adapted version of the Mini BESTest in French compares well to the original version and can be used by French-speaking rehabilitation professionals to examine postural control. By the anchor-based method, the MCID for the HFS-F and DASH was +26 (SD 35) (P < 10−4) and −13 (SD 13) (P < 10−4), respectively, and by the ROC curve, it was +10 to +12 for the Δ-HFS-F and −7.5 to −5 for the Δ-DASH. The area under the ROC curve (AUC) was 0.726 [0.638–0.781] for Δ-HFS-F and 0.768 [0.701–0.83] for Δ-DASH. The correlations between the anchor and delta scores were > 0.38 (P < 10−4). The SEM was 16.2 for the HFS-F and −4.3 for the DASH.
Conclusions
Values below the SEM must be rejected. Our anchor was significantly correlated with the outcome. Methods
The LTPAQ was translated from English to French by forward and backward translation. To assess content validity, 9 adults with physical disabilities read and provided comments regarding the relevance, wording and understanding of the items of the preliminary Canadian-French version of the questionnaire. For test-retest reliability, 37 adults with physical disabilities completed the questionnaire 2 or 3 times at T1 (baseline), T2 (2 days from baseline) and T3 (7 days from baseline). The test–retest reliability was investigated by intra-class correlation coefficients (ICCs), paired t test and Bland and Altman tests.
Results
The translation and the content validation process resulted in a Canadian-French version of the LTPAQ (LTPAQ-CF). Total LTPAQ-CF scores between T1-T2 and T1-T3 featured strong ICCs, 0.90 and 0.75 (P ≤ 0.01). Paired t tests and Bland and Altman analyses confirmed the good reproducibility of results.
Conclusion

The translation and the content validation process resulted in a Canadian-French version of the LTPAQ (LTPAQ-CF). Total LTPAQ-CF scores between T1-T2 and T1-T3 featured strong ICCs, 0.90 and 0.75 (P ≤ 0.01). Paired t tests and Bland and Altman analyses confirmed the good reproducibility of results.
Conclusion

Results
Rasch analysis indicated that: (1) functioning of the 4 response options was acceptable; (2) all items fitted the measured construct [information-weighted (infit) and outlier-sensitive (outfit) mean-square statistics 0.60 to 1.40]; (3) person separation reliability was 0.62 (and Cronbach α = 0.76), item separation reliability was 0.83; (4) on principal component analysis (PCA) on the standardised residuals, the CSD showed borderline but acceptable unidimensionality and no local item dependency. Moreover, as expected, the CSD score showed good correlation with the QUEST 2.0 score (rs = 0.57) and little to fair correlation with the OPUS Upper Extremity Functional Status score (rs = 0.21).
Conclusion
The metric properties of the Slovene version of CSD agree with previous studies. The present study confirms the validity of CSD for measuring patient satisfaction with an upper-limb device, enhances the confidence in this tool for assessing upper-limb prosthesis users, and contributes to further refining the technical quality of this measure.Permalink : ./index.php?lvl=notice_display&id=84115

Prevention of chronicization of low back pain requires accurate detection of at-risk patients. Questionnaires have been validated, including the STarT Back Screening Tool (SBST) and the Örebro Musculoskeletal Pain Screening Questionnaire (OMPSQ). This review aims to compare these questionnaires in terms of predictive value and in terms of aims, to guide the choice in clinical practice.
Methods
This study is a semi-systematic literature review. Studies evaluating at least one of the questionnaires and written between 1997 and October 10th 2017 were selected from Pubmed database. Inclusion criteria were pain duration < 3 months, outcomes including pain, function and/or global recovery. For work outcomes, inclusion criteria were extended to chronic patients. Studies had to provide information on sensitivity, specificity and area under the ROC Curve (AUC).
Results
Twenty-eight studies met our inclusion criteria (7 SBST, 21 original OMPSQ, 3 short OMPSQ). The OMPSQ best predicted a Pain NRS ≥ 3 at 3 months (AUC = 0.64 (0.50–0.78)) and at 6 months (AUC between 0.70 (no confidence interval provided) and 0.84 (0.71–0.97)). The SBST and the OMPSQ are comparable to predict an Oswestry Disability Index ≥ 30% at 6 months. A single study showed no difference between the SBST and the OMPSQ to predict absenteeism ≥ 30 days at 6 months. The two questionnaires cannot be compared for “global recovery” outcomes.
Conclusion
The OMPSQ seems better than the SBST for predicting “pain” and “work” outcomes, the SBST may be better for “function” outcomes. These results should be taken with caution because of the high heterogeneity between studies. It should be noted that the OMPSQ was elaborated with the aim of creating a prognostic tool while the SBST was devised as a treatment-allocating tool and is easier to use in clinical practice.
