Health and social care agencies are encouraged to provide child-centred services that respond to needs that change over time. There is evidence that occupational therapists are concerned about how to respond to issues of harmful behaviour in the home environment whilst promoting child development. This study explored occupational therapists" experiences regarding harmful behaviour and the safety of children with Autistic Spectrum Disorder.
Method
Seven community occupational therapists, employed by a local authority and working in social care, participated in two focus groups.
Findings
An inductive thematic analysis identified five broad themes. These were: 'assessment is complex', 'chidl development and the home environment', 'importance of family-centred services', 'the need for additional services', and 'management of the disabled facilities grant'.
Conclusion
Local authority occupational therapists prioritize supporting families to keep children safe. It is necessary to adapt the home reduce hazards and for alternative interventions to be made available to address behavioural and sensory needs. Services such as sensory integration are lacking, and where they are available access is difficult. Is is common for disabled facilities gratns to fund adaptations; however, management of grants could be improved to meet the needs of these children and their families.Permalink : ./index.php?lvl=notice_display&id=33889

Constraint-induced movement therapy (CIMT) is increasingly recognized as an effective therapy for children with hemiparesis. However, the effectiveness of CIMT outside the standard rehabilitation protocol in clinical settings is less known. The aim of this systematic review was to investigate the effectiveness of CIMT conducted at home or in a group.
Methods
We searched CINAHL, PubMed and ScienceDirect in August 2017 to select articles of studies investigating the impact of CIMT performed at home and in a group on affected upper-limb ability, occupational performance, and quality of life of children. Quality was evaluated with the PEDro scale.
Results
Among 374 reports of studies, 30 met the criteria; 15 examined CIMT at home and 15 in a group. CIMT with the 2 delivery models, at home or in a group, had a positive effect on the affected upper-limb ability and occupational performance. The quality of evidence was high for both these outcomes. However, the evidence was weaker and the results too limited to conclude on the impact on quality of life. The data also suggested that the glove may not be the best type of constraint.
Conclusions
CIMT performed at home or in a group may be a promising intervention for rehabilitation for children with hemiparesis, but more studies on the impact on quality of life are warranted.Permalink : ./index.php?lvl=notice_display&id=80607

The objective of this review was to summarize the current best evidence for the effectiveness, feasibility, user compliance and safety of exercise-based games (EBGs), including virtual reality and interactive video game interventions, for the rehabilitation of individuals with neurological disorders at home.
Material and methods
We identified randomized controlled trials (RCT) evaluating the effects of EBGs in neurological patients in home settings by searching 3 electronic databases (MEDLINE, SCOPUS, CENTRAL Library) from inception to March 2018. All data pertaining to participants, interventions, outcomes, supervision and cost-effectiveness were independently extracted by 2 reviewers. Risk of bias was independently assessed by 2 reviewers.
Results
Reports of 11 RCT studies with heterogeneous populations (i.e., stroke, Parkinson disease and multiple sclerosis) were included in the review. The treatment of experimental groups included EBGs (i.e., commercially available games such as Nintendo Wii or Dance Dance Revolution or custom-designed devices), and control groups received a controlled (i.e., conventional therapy) or uncontrolled (i.e., usual care) intervention. Across studies, EBGs at home tended to have limited effects on upper and lower limbs. We demonstrated an increased risk of participants dropping out of the program or discontinuing training in experimental groups (n = 51 participants) as compared with controls (n = 23 participants). Few adverse events (2 of 6 studies), such as minor musculoskeletal pain, were reported in balance training.
Conclusions
This systematic review reveals that EBGs seem a relevant alternative for rehabilitation at home because the effectiveness of these interventions was at least equivalent to conventional therapy or usual care. These ideas include: (a) finding strategies to achieve satisfactory occupational performance that does not necessarily entail full independence, (b) increased focus on the use and transfer of problem-solving strategies, (c) acknowledging and addressing possible reluctance to use assistive devices, and (d) individually scheduled follow-up visits post occupational therapy.
