Ces deux infections virales se caractérisent par une incubation courte avant l'apparition d'une symptomatologie similaire, généralement constituée d'un syndrome pseudo-grippal et d'une éruption maculo-papuleuse. Elles se distinguent toutefois par leurs complications constituées, pour la dengue de formes hémorragiques engageant le pronostic vital et, pour le chikungunya, de formes invalidantes par le passage à la chronicité des douleurs articulaires.
Dans les deux cas, le diagnostic virologique repose sur la mise en évidence précoce du génome viral et la détection plus tardive des anticorps. Enfin, il n'existe pas de traitement spécifique mais des mesures prophylactiques sont possibles associant vaccination (dengue) et protection individuelle et collective anti-moustique. En conclusion, dengue et chikungunya représentent deux arboviroses à fort potentiel d'émergence dans les pays tempérés par l'expansion du moustique tigre et l'intensification des voyages. Leur recherche en cas de signes cliniques évocateurs chez un individu n'ayant jamais voyagé est vraisemblablement une attitude qu'il faudra systématiquement adopter en période estivale dans un futur proche. However, in this context, genitourinary tract infection and urethral trauma represent specific challenges. Such conditions have been found to significantly deteriorate quality of life and complicate subsequent treatments. Only optimal prevention associated with appropriate treatment allows for the long-term continuation of such bladder management. Here, we discuss the diagnosis and therapeutic and preventive approaches associated with genitourinary tract infection and urethral trauma in this specific population. This “state-of-the-art” article results from a literature review (MEDLINE articles and scientific society guidelines) and the authors’ experience. It was structured in a didactic way to facilitate comprehension and promote the implementation of advice and recommendations in daily practice. Genitourinary tract infection and urethral trauma associated with intermittent catheterization in neurologic patients should be managed with a global approach, including patient and caregiver education, optimal catheterization with hydrophilic-coated or pre-lubricated catheters and adequate use of antibiotic therapy.
