Occupational therapists strive to be mindful, competent practitioners and continuously look for ways to improve practice. Applied behavior analysis (ABA) has strong evidence of effectiveness in helping people with autism achieve goals, yet it does not seem to be implemented in occupational therapy practice. To better understand whether ABA could be an evidence-based option to expand occupational therapy practice, the authors conducted an iterative, multiphase investigation of relevant literature. Findings suggest that occupational therapists apply developmental and sensory approaches to autism treatment. The occupational therapy literature does not reflect any use of ABA despite its strong evidence base. Occupational therapists may currently avoid using ABA principles because of a perception that ABA is not client centered. ABA principles and occupational therapy are compatible, and the two could work synergistically. |

Despite the need for occupational therapy to emphasize client-specific occupational performance, primary emphasis in stroke rehabilitation continues to be on the remediation of client factors and self-care. Such practice leaves many survivors of stroke with continuing performance deficits. Two case studies demonstrate a novel, alternative approach. The Cognitive Orientation to Daily Occupational Performance (CO–OP) treatment is a performance-based, problem-solving approach to developing functional skills that are client centered. CO–OP was used to guide treatment with 2 older women. The findings suggest that the approach has the potential to successfully help clients with stroke achieve their everyday occupational goals and support continued research in this area. This work will lead to a pilot randomized controlled trial. |

Introduction Setting goals with individuals with acquired brain injury may be challenging due to impairments in cognition and communication. The purpose of this study was to explore how occupational therapists' communication behaviours during goal-setting with individuals with traumatic brain injury facilitated and hindered this process.
Method This exploratory study used a conversation analysis inspired approach and frequency calculations to analyse and interpret videotaped goal-setting sessions. Sequences of dialogue leading to, and distracting from, problem identification, a key step in goal-setting, were identified and analysed. Specific therapist behaviours that facilitated or hindered problem identification were subsequently distinguished.
Results Acknowledgements and affirmations, open-ended questions about specific tasks and reflective listening, were found to lead to problem identification by the client (facilitators). Instances of disconnections were characterized by a single theme, ‘lack of uptake.' Examples of these hindrances to goal-setting included, abrupt topic shifts, lack of acknowlegement and failure to explore what the client said.
Conclusion Clinicians should consider their language use during goal-setting interviews and aim to utilize conversational behaviours that are facilitative whilst minimizing those that distract to optimize their client's engagement during the problem identification phase of goal-setting. |
