Santé mentale # Théorie de la connaissanceMots-clés : Santé mentale Connaissance Résumé : "As the integration of health and social care services progresses in the mental health sector, there is concern that mental health social workers are disadvantaged, relative to health professionals, because they cannot identify the knowledge base for their practice. This paper argues that this concern is partly the product of assuming that the knowledge base has to be premised upon randomized controlled trials. Instead, it proposes a non-hierarchical framework based on that developed in health research in Canada by Upshur and colleagues that generates a typology of knowledge which is congruent with the main forms of inquiry that are relevant to mental health social work practice. The framework recognizes the contribution of randomized controlled trials to the knowledge base but also validates knowledge drawn from qualitative, epidemiological, practitioner and user knowledge. It is argued that the framework profiles a wider knowledge base than that promoted by conventional evidence-based practice, and also could be a basis for future development of the research agenda in mental health social work." It is suggested that when working with severely deprived and traumatized children, the therapist should hold in mind the lacking of rapport with primary caretakers, and the need of the child to form meaningful and intimate relations in his or her future life. Psychoanalytically based thinking enriches the treatment process by offering new ways for understanding of the patient’s needs, aiming to reach the child’s subjective experience and re-establishing his sense of self and a meaningful rapport with an other. The concept of ‘potential space’ and the area of experience (Winnicott, 1953/1975) will be described with elaboration on its variety and use in theory and practice, focusing on two major themes: the dialectics of knowing and not knowing, and the search for selected material. A clinical illustration is presented to show these dynamics as they appear in the clinical process and the therapeutic session. The material was selected from supervision of graduate social work students involved in therapeutic work with children." Drawing on recent doctoral research, this paper suggests that a particular type of reflective practice—holistic reflective practice—has the potential to encourage thoughtful and creative practice capable of addressing the challenges of contemporary child-care practice. Findings from this research indicate that for holistic reflective practice to be facilitated, the interdependence of the practitioner, team and organizational contexts needs to be recognized. Practitioners need to work within safe containing contexts characterized by: clear organizational and professional boundaries; multifaceted reflective forums; collaborative and communicative working practices; and open and ‘contextually connected’ managers. Drawing on these findings and theorizing them in relation to Bion’s concept of containment, the paper concludes by proposing a model of containment for the promotion of reflective practice."
