Patients divided into two groups of treatment strategy. Bilateral segmental fixation group’s results were similar to alternate fixation group. Although quality of life or cosmetic evaluation of patients not involved in this study, alternate fixation provides similar results as bilateral segmental fixation in adolescen idiopathic scoliosis. Dans cet article, nous décrivons la validation d’une technique HRM ( high resolution melting) permettant un génotypage rapide de ce variant. Trente-quatre patients de génotype connu [5 sujets sains (U/U), 12 hétérozygotes (U/A) et 17 homozygotes (A/A) - A : allèle atypique du gène BCHE, U : allèle sauvage du gène BCHE -] ont été analysés avec la technique HRM. La répétabilité et la reproductibilité de la technique ont également été étudiées. Une modélisation in silico de l’étape de fusion a été réalisée afin d’évaluer l’impact potentiel des autres variations décrites au sein de la séquence amplifiée sur les performances diagnostiques de la technique. La technique présentée constitue une alternative aux autres approches méthodologiques pour la détection du variant atypique du gène BCHE. Elle présente de très bonnes performances en termes de répétabilité/reproductibilité, les coefficients de variations des Tm des trois génotypes étant inférieurs ou égaux à 0,21 % lors de l’étude de reproductibilité. Par ailleurs, le risque d’erreur de classification lié à la présence des autres variations nucléotidiques décrites au sein de la séquence amplifiée est faible. This study reviews our results in consecutive patients having had a bilateral sacroiliac joint fusion over a five-year period. Fifteen patients had bilateral sacroiliac joint fusions with 13 having concurrent lumbosacral fusions. The modified posterior midline fascial splitting approach, first described by Belanger was utilized. Patients were followed for an average of 30.3 months. There were no infections, neurovascular injuries, lasting morbidity or deaths. One non-union of a sacroiliac joint (7%) occurred, which after revision was satisfactory. There was a statistically significant drop in pain (p = 0.01488) using the VAS, and patient satisfaction rates were 86%. With all those patients saying they would have the surgery again for the same result. There was no significant increase in functionality. Patients needing bilateral sacroiliac joint fusions frequently fall into the “failed back” category, and it is important to evaluate both the sacroiliac joints and the lumbosacral spine for potential pain generators. This study shows that by treating all the pain generators in both areas there were significant decreases in pain, low complications, low re-operation rates, and high patient satisfaction scores.
