L’analyse de ce cas est l’occasion de discuter de la découverte d’hallucinations visuelles postopératoires. Dans ce contexte, le dépistage et l’analyse diagnostic de ce symptôme, fréquent en unité périopératoire gériatrique, sont importants. L’existence d’hallucinations dans les suites d’une chirurgie peut entraîner anxiété et agitation. Les étiologies multiples possibles dans ce cas ont permis d’explorer plusieurs hypothèses et de démontrer l’importance de la temporalité d’évolution de ce symptôme. Les hallucinations doivent être recherchées et explorées. La prise en charge médicamenteuse et non médicamenteuse des hallucinations dépend de leur étiologie et nécessite une attention particulière dans ce contexte d’anesthésie récente. The study retrospectively evaluated 16 patients who, between 2007 and 2014, developed pseudarthrosis following treatment for a femoral neck fracture and who were treated with DHS- osteosynthesis, after a valgus subtrochanteric osteotomy. Postoperative clinical evaluation of the patients was done? using the Harris Hip Scoring (HHS) system.
Union of both the fracture and the osteotomy site was achieved in 17.2 weeks (range: 14-24 weeks) in all patients. The average Pauwels angle decreased from 72o (range 62–80) preoperatively to 26o (range 20–50) postoperatively. All fractures were Pauwels type III preoperatively and 4 type II and 12 type I postoperatively. The average HHS increased from 26 (range 18–34) preoperatively to 85 (range 68–94) postoperatively. Of the patients who were followed up for a mean duration of 3.1 years (range: 1-5 years), four had 1-cm shortening. No patient developed postoperative AVN of the femoral head.
For patients with non-union after femoral neck fracture, DHS-osteosynthesis after valgus osteotomy is a method with a shorter learning curve, which can be successfully performed.Permalink : ./index.php?lvl=notice_display&id=92185

Hip fractures are associated with causes and consequences of frailty such as osteoporosis, frequent falls, low body mass index, multiple medications and cognitive impairment. The aim of our study is to assess the value of ASA grade and Edmonton frailty score in the outcome of treatment of fracture neck of femurs in elderly patients.
192 patients admitted with fracture neck of femur were included in the study. The mean age was 79.23 years .120 patients had ASA grade 3, 56 patients had ASA grade 2 and 16 patients had ASA grade 1.The frailty index was calculated using Edmonton scoring index. Ninety four patients (49%) had low frailty score and 88 patients (51%) had a high frailty score of more than 10.All patients were followed up 4 weeks and one year after the surgery.
In conclusion the patients with frailty scores and ASA grade have got more chance of developing wound infection. They also have got higher incidence of mortality and morbidity following fracture neck of femur.Permalink : ./index.php?lvl=notice_display&id=92213
