Common adverse effects of TKA were infection, loosening, pain, instability and hyper- coagulation disorders. Serious adverse effects related to TKA implants were not reported. The survival ratios and patient-reported outcome measures seem to confirm these good results. In contrast with chromium and cobalt, no significant differences were reported in the nickel, molybdenum and titanium concentrations. No significant differences between the hypoallergenic and standard TKA implants were found in terms of adverse effects, survival ratios and PROMs. A causal relationship between the common adverse effects and the different metals is unlikely. Due to the heterogeneity of the TKA implants used, no firm conclusions could be made. Further research with longer follow-up studies are needed to find possible adverse effects and differences. The survival rate of the implants was close to 95% at follow-up.

Introduction
Le but de l’étude est d’évaluer les facteurs de risque des ruptures du ligament croisé antérieur (LCA) chez les skieurs alpins de haut niveau.
Méthode
Ont été inclus dans cette étude, les skieurs et skieuses des équipes de France de ski alpin ayant présenté une rupture du LCA entre avril 2002 et avril 2010. Il s’agit d’une étude rétrospective portant sur 8 saisons comprenant 2 olympiades. Un mail a été adressé à chaque skieur concerné avec un questionnaire à remplir (un questionnaire par rupture ligamentaire).
Résultats
Quarante-deux skieurs ont été inclus dans l’étude (24 skieuses, 18 skieurs) : 55 % des blessures sont survenues par temps ensoleillé et 22 % par temps couvert, 52 % sur neige dure-compacte et 24 % sur neige injectée, 60 % sur piste bien préparée. La descente et le géant ont induit 74 % des blessures. Les athlètes se sont surtout blessés en décembre et janvier (52,5 %). L’échauffement était globalement bien réalisé (83 %), les skieurs en forme et bien concentrés. La période de l’accident se caractérisait par de nombreux déplacements, un stress et un nombre de courses importantes. Retenons 27 cas de non-déclenchement des fixations malgré un réglage selon les normes internationales.
Discussion
L’évolution du matériel de ski alpin a nettement augmenté le nombre de ruptures du pivot central en majorant les contraintes au niveau du genou.
Conclusion
Les facteurs environnementaux et intrinsèques semblent désormais bien maîtrisés. N’existe-t-il pas un retard d’adaptation du skieur à un matériel en constante évolution et de plus en plus performant ?

Material and method
We included the French team's skiers who suffered from an ACL total rupture between April 2002 and April 2010. It was a retrospective study about 8 seasons of ski and including 2 Olympiads. We sent by mail to each concerned skier a questionnaire. They had to answer to one questionnaire by rupture in the considered period.
Results
Forty-two answers (24 from girls, 18 from men) were collected. Fifty-five percent of injuries occurred by sunny weather and 22% by overcast weather, 52% on compact snow and 24% on ice-cold snow, 60% on well-prepared ski run. Downhill and Giant Slalom were responsible for 74% of injuries. Athletes especially injured themselves on December and January (52.5%). The warm-up was globally well done (83%), the skiers not tired and well concentrated. The injuries occurred mostly during a period with many travels and races, lot of stress. It is necessary to hold 27 cases of non-release of bindings while international standard were respected.
Discussion
The evolution of alpine skiing equipment clearly increased the number of ACL injuries, increasing forces on the knee.
Conclusion
From now on, environmental and intrinsic factors seem to be well controlled. We can wonder if it exists a delay of adjustment of skiers face to a constantly evolving equipment which becomes more and more successful.Permalink : ./index.php?lvl=notice_display&id=34506

Significant valgus deformity needs to be treated surgically to improve appearance, gait and function of the patient. The purpose of this prospective study was to evaluate the results of supracondylar “V” osteotomy as a surgical technique for correction of the valgus knee deformity.
This study was conducted in a tertiary level teaching hospital and 30 cases were included in the study. For all the patients deformity was assessed using ana- tomical tibiofemoral angle, mechanical axis deviation and intermalleolar distance preoperatively and post- operatively.
The average age of our patients was 13.7 years and the average follow up was3.29 years (1.39-14.22 yrs). Clinically the average value of intermalleolar distance preoperatively was 16cm and 3.2 cm postperatively. Average pre-operative tibiofemoral angle was 23° and the average postoperative angle was 6 0 which was found to be statistically significant using the Paired t test (p<0.005). The average value of preoperative mechanical axis deviation was 3.1 cm which decreased to an average value of 1.1 cm postoperatively.
The results with this technique have been encouraging. The advantages of this technique are low morbidity, good stability allowing early ambulation, ability to adjust alignment postoperatively by casting and no need for internal fixation. Few studies have been conducted on osteotomies that do not require internal fixation and are inherently stable. This technique has the advantage of practically no occurrence of any infection or a second surgery to remove hardware in children and adolescents. Since no specialized instrumentation, image intensifier and implants are required, it is cost effective and can be used in any primary care or district level surgical setup in a developing country like ours.Permalink : ./index.php?lvl=notice_display&id=96627

Nous présentons le cas d’un volleyeur professionnel, ayant consulté pour gonalgie postéro-médiale apparue lors de la réception en hyperextension pendant un match de volley-ball. L’IRM réalisée a permis de poser le diagnostic de tendino-bursite isolée par tendinopathie du faisceau réfléchi du semi-membraneux, avec épanchement liquidien de la bourse semi-membraneux–ligament collatéral médial. Quelques points clés de l’anatomie du complexe distal d’insertion du muscle semi-membraneux sont à retenir ; les cinq expansions tendineuses dont fait partie le faisceau réfléchi sont à analyser en relation étroite avec les deux bourses synoviales qui leur sont associées. En effet, la distension liquidienne de la bourse semi-membraneux–ligament collatéral médial, récemment décrite, doit être considérée comme une lésion sentinelle et pourrait orienter le radiologue vers une atteinte du complexe semi-membraneux. Le mécanisme physiopathologique de l’atteinte du complexe distal du muscle semi-membraneux distingue deux types d’atteinte : l’hypersollicitation et l’hyperextension tendineuses du sujet jeune et sportif seraient responsables respectivement des atteintes isolées des tendons direct et réfléchi. La tendinopathie du sujet d’âge moyen serait volontiers secondaire à des microtraumatismes répétés.
Abstract
We present the case of a professional volleyball player, referred for posteromedial knee pain following a competition. MRI diagnosed an anterior arm of the semi-membraneous tendinitis associated with a semimembranosus–tibial collateral ligament bursitis. Some key points of the anatomy of the distal semimembranosus complex must be identified; five tendinous arms (including the anterior, or tibial arm) are to be seen with regard to the two synovial bursae associated. Indeed, a fluid distension of the semimembranosus–tibial collateral ligament, recently described, must be considered as a sentinel lesion and could turn the radiologist toward an injury of the distal semimembranosus complex. The physiopathological mechanism of the injury of the distal semimembranosus complex distinguishes two kinds of lesion: the tendinous overstressing and the hyperextension of the young and athletic subject will be responsible respectively for isolated tendinitis of the direct and reflected tendons. Nous présentons le cas d’un volleyeur professionnel, ayant consulté pour gonalgie postéro-médiale apparue lors de la réception en hyperextension pendant un match de volley-ball. L’IRM réalisée a permis de poser le diagnostic de tendino-bursite isolée par tendinopathie du faisceau réfléchi du semi-membraneux, avec épanchement liquidien de la bourse semi-membraneux–ligament collatéral médial. Quelques points clés de l’anatomie du complexe distal d’insertion du muscle semi-membraneux sont à retenir ; les cinq expansions tendineuses dont fait partie le faisceau réfléchi sont à analyser en relation étroite avec les deux bourses synoviales qui leur sont associées. En effet, la distension liquidienne de la bourse semi-membraneux–ligament collatéral médial, récemment décrite, doit être considérée comme une lésion sentinelle et pourrait orienter le radiologue vers une atteinte du complexe semi-membraneux. Le mécanisme physiopathologique de l’atteinte du complexe distal du muscle semi-membraneux distingue deux types d’atteinte : l’hypersollicitation et l’hyperextension tendineuses du sujet jeune et sportif seraient responsables respectivement des atteintes isolées des tendons direct et réfléchi. La tendinopathie du sujet d’âge moyen serait volontiers secondaire à des microtraumatismes répétés.
Abstract
We present the case of a professional volleyball player, referred for posteromedial knee pain following a competition. MRI diagnosed an anterior arm of the semi-membraneous tendinitis associated with a semimembranosus–tibial collateral ligament bursitis. Some key points of the anatomy of the distal semimembranosus complex must be identified; five tendinous arms (including the anterior, or tibial arm) are to be seen with regard to the two synovial bursae associated. Indeed, a fluid distension of the semimembranosus–tibial collateral ligament, recently described, must be considered as a sentinel lesion and could turn the radiologist toward an injury of the distal semimembranosus complex. The physiopathological mechanism of the injury of the distal semimembranosus complex distinguishes two kinds of lesion: the tendinous overstressing and the hyperextension of the young and athletic subject will be responsible respectively for isolated tendinitis of the direct and reflected tendons.
