To investigate the prevalence of pregnancy-related low back pain (PRLBP) in women in Turkey, identify the factors associated with PRLBP and predict the risk of PRLBP.
Materials and methods
This cross-sectional study included a total of 1500 pregnant women admitted to a prenatal care clinic in a secondary care hospital in Turkey between August 2011 and September 2014. All participants were asked to complete a survey questionnaire. The pregnant women who reported recurrent or continuous pain in the lumbar spine or pelvis for more than 1 week were offered a clinical examination for PRLBP by the spine physiatrist. The main outcome measure was the presence of PRLBP. We collected data on sociodemographic factors, previous obstetric history, daily habits, history of LBP, and functional disability scores as assessed by the Oswestry Disability Index (ODI).
Results
The mean age of the 1500 women was 26.5±5.5 years. The prevalence of PRLBP was 53.9%, mostly in the third trimester. Women with PRLBP in the third trimester were more disabled than those in the first and second trimesters (mean ODI 40.0±16.7 vs. 34.9±19.2 and 37.4±15.3, respectively). Risk factors of PRLBP were history of LBP, PRLBP, and menstruation-related LBP as well as no housework assistance (OR=5.394, 95% CI: 3.128–9.300, P<0.001; 3.692, 2.745–4.964, P<0.001; 2.141, 1.563–2.932, P<0.001; 1.300, 1.029–1.64, P=0.028, respectively).
Conclusion
This cross-sectional study is the largest study of PRLBP in the literature and showed that about 1 in 2 women have PRLBP in any stage of pregnancy. History of LBP related and unrelated to previous pregnancy and menstruation are strong risk factors for PRLBP. No study has examined the associations among physical fitness, cognition, and driving performance in people with HF.
METHOD. Eighteen people with HF completed a physical fitness assessment, a cognitive test battery, and a validated driving simulator scenario.
RESULTS. Partial correlations showed that poorer physical fitness was correlated with more collisions and stop signs missed and lower scores on a composite score of attention, executive function, and psychomotor speed. Cognitive dysfunction predicted reduced driving simulation performance.
CONCLUSION. Reduced physical fitness in participants with HF was associated with worse simulated driving, possibly because of cognitive dysfunction. Introduction
Le but de l’étude est d’évaluer les facteurs de risque des ruptures du ligament croisé antérieur (LCA) chez les skieurs alpins de haut niveau.
Méthode
Ont été inclus dans cette étude, les skieurs et skieuses des équipes de France de ski alpin ayant présenté une rupture du LCA entre avril 2002 et avril 2010. Il s’agit d’une étude rétrospective portant sur 8 saisons comprenant 2 olympiades. Un mail a été adressé à chaque skieur concerné avec un questionnaire à remplir (un questionnaire par rupture ligamentaire).
Résultats
Quarante-deux skieurs ont été inclus dans l’étude (24 skieuses, 18 skieurs) : 55 % des blessures sont survenues par temps ensoleillé et 22 % par temps couvert, 52 % sur neige dure-compacte et 24 % sur neige injectée, 60 % sur piste bien préparée. La descente et le géant ont induit 74 % des blessures. Les athlètes se sont surtout blessés en décembre et janvier (52,5 %). L’échauffement était globalement bien réalisé (83 %), les skieurs en forme et bien concentrés. La période de l’accident se caractérisait par de nombreux déplacements, un stress et un nombre de courses importantes. Retenons 27 cas de non-déclenchement des fixations malgré un réglage selon les normes internationales.
Discussion
L’évolution du matériel de ski alpin a nettement augmenté le nombre de ruptures du pivot central en majorant les contraintes au niveau du genou.
Conclusion
Les facteurs environnementaux et intrinsèques semblent désormais bien maîtrisés. N’existe-t-il pas un retard d’adaptation du skieur à un matériel en constante évolution et de plus en plus performant ?

Material and method
We included the French team's skiers who suffered from an ACL total rupture between April 2002 and April 2010. It was a retrospective study about 8 seasons of ski and including 2 Olympiads. We sent by mail to each concerned skier a questionnaire. They had to answer to one questionnaire by rupture in the considered period.
Results
Forty-two answers (24 from girls, 18 from men) were collected. Fifty-five percent of injuries occurred by sunny weather and 22% by overcast weather, 52% on compact snow and 24% on ice-cold snow, 60% on well-prepared ski run. Downhill and Giant Slalom were responsible for 74% of injuries. Athletes especially injured themselves on December and January (52.5%). The warm-up was globally well done (83%), the skiers not tired and well concentrated. The injuries occurred mostly during a period with many travels and races, lot of stress. It is necessary to hold 27 cases of non-release of bindings while international standard were respected.
Discussion
The evolution of alpine skiing equipment clearly increased the number of ACL injuries, increasing forces on the knee.
Conclusion
From now on, environmental and intrinsic factors seem to be well controlled. We can wonder if it exists a delay of adjustment of skiers face to a constantly evolving equipment which becomes more and more successful.Permalink : ./index.php?lvl=notice_display&id=34506

To undertake a critical review of literature on use of legal and illegal psychoactive substances (PAS) in persons with spinal cord injury (SCI) before and after trauma.
Material and methods
Hundred and five articles published between 1980 and 2014 on alcohol and drug use in persons with SCI before and after trauma were retrieved from the PubMed and PsycInfo search engines.
Results
Before injury, 25% to 96% of people with SCI reported using alcohol, while 32% to 35% had used illegal drugs. At the time of injury, 31% to 50% of individuals with SCI were intoxicated with alcohol, 16% to 33% with drugs and 26% with a combination of drugs and alcohol. Among those reporting PAS use before injury, up to 50% stated that they had reduced their use during active rehabilitation, during which time only 6% consumed psychoactive substances for the first time. A variety of risk factors are associated with consumption subsequent to spinal cord injury: personality alteration (impulsiveness, aggressiveness), posttraumatic depression, poor coping skills, lack of social support and pain. PAS use can affect the process of rehabilitation, diminish the effectiveness of medication and result in various medical complications.
Discussion/conclusions
Few studies have explored the use of alcohol, drugs and psychoactive medications before SCI and during active rehabilitation. To our knowledge, no study has analyzed the evolution of PAS use after hospital discharge, even though return home is associated with new stressors that may trigger risky behaviors. It should be a priority, as early as possible during rehabilitation, to detect persons at risk of developing PAS abuse.
Objectif
Revue de la littérature quant à l’usage de substances psychoactives (SPA) illicites avant et suite au traumatisme chez les personnes ayant une lésion à la moelle épinière (LM).
Matériel et méthode
Cent-cinq articles entre 1980 et 2014, en lien avec l’usage de SPA avant et suite au traumatisme chez les personnes LM. Les articles ont été obtenus par les moteurs de recherche PubMed et PsycInfo.
Résultats
Avant la lésion médullaire, entre 25 et 96 % des personnes LM affichaient une consommation d’alcool et 32 à 35 % de substances illicites. Au moment de l’accident, entre 31 et 50 % des individus LM auraient été intoxiqués par l’alcool, 16 à 33 % aux substances illicites et 26 % à la fois par l’alcool et les substances illicites. Parmi les individus qui consommaient des SPA avant le traumatisme, jusqu’à 50 % des personnes ont diminué leur consommation durant la phase de réadaptation alors que 6 % se sont initiées à la consommation. Divers facteurs de risque sont associés à la consommation chez les personnes LM comme la personnalité (impulsivité, agressivité), un état dépressif post-traumatique, les difficultés d’adaptation à la lésion, le réseau social de même que la douleur. Cette consommation peut affecter le processus de réadaptation, l’efficacité de la médication et engendrer diverses complications médicales.
Discussion/conclusion
Peu d’études semblent avoir exploré à la fois l’usage d’alcool, de substances illicites et de médicaments psychotropes avant le traumatisme et lors de la réadaptation fonctionnelle, et aucune n’a tenté d’explorer l’évolution de la consommation lors du retour à domicile, période qui présente pourtant de nouvelles contraintes et de nouveaux agents stresseurs susceptibles d’engendrer des comportements à risque.
