Many studies have shown that systemic and local infections cause fibrinolytic activities. The purpose of this study was to determine the role of serum D-dimer in the diagnosis of periprosthetic joint infection (PJI) and the timing of reimplantation. The patients who underwent primary and revision knee or hip arthroplasty between July 2018 and May 2019 were prospectively evaluated. All surgeries were performed by the same surgeon. 60 of 71 patients who met our criteria were included in the study, comprising of 27 primary arthroplasties (PA), 21 two-stage septic revision (SR) and 12 aseptic revision (AR). We analyzed D-dimer, C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) levels. The values of the septic group were evaluated as two groups; Group 1 (before the first stage) and Group 2 (before the second stage) to investigate the role of D-dimer in determining the timing of reimplantation. D-dimer level was significantly higher in the SR group compared to PA and AR groups (p<0.001 and p=0.045). CRP and ESR levels were significantly higher in the SR group compared to PA and AR groups. There was no statistically significant difference between preoperative D-dimer levels of Group 1 and Group 2 (p=0.2). Serum D-dimer level can be used as an easy and inexpensive test to support the diagnosis of PJI. But the D-dimer level is not useful to determine the timing of reimplantation. Nous avons élaboré un questionnaire visant à aider le diagnostic positif des entorses externes bénignes récentes de la cheville (questionnaire EEBRC) par les pharmaciens officinaux.
Objectif
Analyser les qualités métriques du questionnaire EEBRC.
Méthodes
La performance diagnostique d’un ensemble de 10 questions (items), regroupées dans une section « examen initial » du questionnaire EEBRC, a été testée comparativement aux données d’une étude prospective randomisée de 388 patients pris en charge aux urgences traumatologiques pour une entorse externe de la cheville (cohorte TALOS). Dans cette dernière, après un suivi de 1 an, le diagnostic final d’entorse bénigne, moyenne et grave de la cheville a été posé chez respectivement 28 %, 22 % et 38 % des patients, 12 % présentant un diagnostic différentiel.
Résultats
Le questionnaire à 10 items avait une spécificité de 98,6 % ; une spécificité comparable (98,4 %) était conservée par une version simplifiée à 7 items. La sensibilité de ces deux versions était de 1,7 % et 1,5 % respectivement. Le meilleur équilibre entre spécificité et sensibilité (86,7 % et 29,2 %, respectivement) était associé à une version à 5 items.
Conclusions
En raison de la haute spécificité du questionnaire EEBRC à 7 items, un résultat positif permet d’affirmer le diagnostic d’entorse externe bénigne de la cheville avec un risque d’erreur faible. Ce questionnaire n’est pas adapté à la reconnaissance et à la classification de toutes les catégories de gravité de l’entorse. De futures études visant à affiner ce questionnaire et à tester prospectivement sa validation en conditions réelles d’utilisation méritent d’être réalisées.
Introduction
We developed a questionnaire aimed at helping the positive diagnosis of recent benign external ankle sprains (EEBRC questionnaire) by community pharmacists.
Objective
The objective was to test the diagnostic accuracy of the EEBRC questionnaire.
Methods
The diagnostic accuracy of a subset of ten questions (items), which constituted the “initial examination” section of the EEBRC questionnaire, was tested versus data from a prospective randomized study enrolling 388 patients which were managed in an emergency department for an external ankle sprain (the TALOS cohort). In this cohort, after 1-year follow-up, the final diagnosis of mild, moderate and severe ankle sprain was made for 28%, 22% and 38% of patients respectively, 12% having an alternative diagnosis.
Results
The 10-items questionnaire had 98.6% specificity; a comparable 98.4% specificity was found with a simplified 7-items questionnaire. The sensitivity of these questionnaires was 1.7% and 1.5% respectively. The best specificity/sensitivity ratio (86.7% and 29.2%, respectively) was associated with a 5-items version of the questionnaire.
Conclusions
Based on the high specificity of the 7-items EEBRC questionnaire, a positive result rules in the diagnosis of a benign external ankle sprain with a low error risk. This questionnaire is not appropriate to diagnose and classify all categories of ankle sprain severity.
