Demographic data on the participants’ cycling specialty and training level was noted. Clinical information concerning knee pain intensity, location and occurrence were collected using a questionnaire.
Our results show 7 different overuse injuries were identified. The prepatellar friction syndrome accounted for the majority of these overuse injuries (46%), while medial plica syndrome (15%), biceps femoris tendinopathy (7.5%), patellar tendinopathy (9.4%), infrapatellar plica friction syndrome (7.5%), infrapatellar fat pad impingement (5.7%) and iliotibial band syndrome (3.7%) were other causes of knee pain in these athletes. Most authors believe that partial ACL resection is the treatment of choice in symptomatic MD of the ACL. This study concerns complete ACL resection as a treatment of mucoid degenerated ACL.
Thirteen patients were treated with complete ACL resection for symptomatic MD of the ACL from 2006-2016. Symptoms included limited range of motion (ROM) and posterior knee pain.
Preoperative and postoperative results of eleven patient were compared. Postoperative recovery was four weeks on average. All patients (100%, n=11) reported an improvement in pain. All patients with a preoperative limited ROM (100%, n=9) reported a return to a normal (subjective) ROM. Eight patients (72.7%, n=8) reported an improvement of knee instability, while three patients (27.3%, n=3) experienced an unchanged knee stability. All patients (100%, n=11) reported a good to very good surgery satisfaction. Preoperative and postoperative KOOS questionnaires were compared and showed improvement in all categories.

Résumé : Mucoid degeneration (MD) of the anterior cruciate ligament (ACL) is a benign knee pathology, which is often indicated for surgical treatment if symptomatic. Most authors believe that partial ACL resection is the treatment of choice in symptomatic MD of the ACL. This study concerns complete ACL resection as a treatment of mucoid degenerated ACL.
Thirteen patients were treated with complete ACL resection for symptomatic MD of the ACL from 2006-2016. Symptoms included limited range of motion (ROM) and posterior knee pain.
Preoperative and postoperative results of eleven patient were compared. Postoperative recovery was four weeks on average. All patients (100%, n=11) reported an improvement in pain. All patients with a preoperative limited ROM (100%, n=9) reported a return to a normal (subjective) ROM. Eight patients (72.7%, n=8) reported an improvement of knee instability, while three patients (27.3%, n=3) experienced an unchanged knee stability. All patients (100%, n=11) reported a good to very good surgery satisfaction. Preoperative and postoperative KOOS questionnaires were compared and showed improvement in all categories.

Vertical ground reaction forces during sit-to-stand, stair descending and squatting were assessed by force plates. Patients were asked to complete the KOOS, OKS and 2011 KSS.
The symmetry-ratios during sit-to-stand, squat and stair-descent improved significantly. Preopera-tive quadriceps-force was positively correlated with KOOS-Symptoms (r=0.583, p=0.037). The pre-operative load-symmetry ratio during STS was negatively correlated with improvement in KOOS Pain (r=-0.675, p=0.011) and Symptoms (r=-0.674, p=0.008). In deep flexion, preoperative bodyweight ratio was positively correlated with postoperative OKS (r=0.601, p=0.039), KSS-Satisfaction (r=0.675, p=0.011) and improvement in KSS-Satisfaction (r=0.684, p=0.029).

This results in a 2% dislocation rate, which is lower than the 3.1% cumulative rate reported earlier in literature. Men and women were distributed equally, with a mean age of 56 years old. The indication for RHTKA among the six dislocations was revision in 4 cases and primary arthroplasty for the other 2 cases.
Analysis revealed that the main mechanism of hinge dislocation was forced knee flexion with concomitant extensor mechanism insufficiency (4/6 cases). The second cause was unscrewing of the locking pin (2/6 cases). This is probably caused by the screw home mechanism that results in a counterclockwise torque -and therefore a loosening- effect on locking bolts, specifically in right-sided RHTKA. Obesity probably predisposes to hinge dislocation since 83% of patients in this series were obese (BMI, range 34 to 52). The findings of this study suggest that dislocation of RHTKA is a rare complication that could happen to obsese patients without an adequate extensor mechanism.

The results of the Ilizarov bone transport technique were retrospectively evaluated in ten patients. All underwent an extensive resection and reconstruction aiming at an ankle arthrodesis. The outcome was registered by clinical and radiographic examination as proposed by Paley’s functional and bone results classification.
A good healing at the level of the docking site could be obtained in all patients but with a re- intervention in 8 of the 10. In 5 of these patients, re-intervention with a transcalcaneal nailing leaded to the final healing. Other options are debridement of the docking site (2 patients) and a new Ilizarov procedure (1 patient).
If patients are prepared to participate in a long-term treatment with the risk of multiple interventions a reconstruction can be performed, resulting in a limb with an acceptable function, allowing all activities of daily life and even a professional occupation. To obtain this final result with a definite union at the docking site a secondary retrograde intramedullary nailing is considered a valuable and safe procedure.Permalink : ./index.php?lvl=notice_display&id=91997

Résumé : A significant number of patients with pilon tibial fractures develop complications, the most devastating being a combination of infection and non-union with bone loss.

PVNS is a rare benign disease of the synovial tissue. It is especially rare in ankles of children, with only 15 cases reported in literature.
The girl presented with swelling and pain in the left ankle since 4 weeks. The tentative diagnosis was made after a joint puncture and a MRI scan. A synovial mass with a brown-yellowish appearance was seen during the excisional biopsy. After removing the entire mass and without adjuvant therapy, no recurrence was detected after 12 months.
As it is very rare in children, PVNS is easily misdiagnosed. Early diagnosis is important to prevent bone and cartilage damage. A review of the clinical, radiological and therapeutical features of PVNS are presented.Permalink : ./index.php?lvl=notice_display&id=91998
