A man in his 70s presented with confusion, fever (40 °C) for four days, generalized pruritus, and a two-week macular abdominal rash. His history included hypertension treated with atenolol and thiazide diuretics and off-label lamotrigine for fibromyalgia for four months, though adherence was inconsistent.Laboratory investigations showed hemoglobin 166 g/L, hematocrit 56%, leukocytosis (16 ×109/L), eosinophilia (3×109/L), and lymphocytosis (5×109/L).
