L'hospitalisation a longtemps été considérée comme la modalité de soins de référence des troubles de l'alcoolisation. Elle peut en effet avoir de nombreux avantages sur le plan conceptuel, mais c'est une modalité de soins coûteuse et dont les longs délais d'attente lui font préférer le soin ambulatoire. Qui plus est, les modalités (ambulatoires ou résidentielles) du soin en alcoologie semblent assez peu différer en termes d'efficacité dans les revues de littérature. Il semblerait ainsi utile de déterminer quelles sont les indications préférentielles de l'hospitalisation pour sevrage. Le choix d'une hospitalisation programmée pour sevrage doit être avant tout guidé par les contre-indications au sevrage ambulatoire. Néanmoins, elle dépend aussi beaucoup de l'avis du patient, élément essentiel de la réussite du projet auquel il doit adhérer, puisqu'il en est l'acteur principal. D'autres situations vont nécessiter la mise en oeuvre urgente de soins en milieu hospitalier, quel que soit le stade motivationnel ou le type de trouble de l'alcoolisation sous-jacent.
ENGLISH:
Inpatient treatment has long been considered the reference in the treatment of alcoholism. It may indeed have many conceptual advantages, but practically it is a method of treatment with high costs and long waiting period. Moreover, reviews of studies evaluating effectiveness of treatment settings for alcohol dependence suggest that no significant differences exist between inpatient and outpatient programs. Therefore, it seems useful to determine indications of inpatient detoxification programs. As we may see, the choice of inpatient detox should be primarily guided by the contra-indications of outpatient detox. However, it also depends very much on patients' preference, essential to success, since they are the lead actors. Parmi les approches psychologiques ayant montré leur efficacité, l'intervention brève est particulièrement adaptée à la prise en charge en médecine générale. Elle vise à accroître la motivation au changement des patients après le repérage d'une consommation excessive d'alcool. Pour un patient alcoolodépendant désirant s'engager vers l'abstinence, le traitement pharmacologique est d'abord constitué d'une période, brève, de sevrage, à l'aide de benzodiazépines en cas de syndrome de sevrage cliniquement significatif. Il est ensuite représenté par la prévention de la rechute, pour laquelle il n'existe toujours que trois médicaments validés, l'acamprosate, la naltrexone et le disulfirame.
ENGLISH:
Amongst validated psychological treatments, brief intervention appears to be the best suited for primary care. It aims at increasing motivation for change after a screening for an alcohol use disorder. The first therapeutic step for an alcohol-dependent patient aiming at abstinence is alcohol detoxification. At this stage, benzodiazepines should be briefly prescribed if the patient manifests clinically significant withdrawal symptoms.
