Even though predictive risk classifications are a widely accepted tool to define a suitable treatment protocol a classification is still missing considering the difficulty in treating the causative pathogen antibiotically. In this study, we present and evaluate a new predictive risk stratification for prosthetic joint infections in 120 cases, treated with a two-stage exchange. Treatment outcomes in 120 patients with proven prosthetic joint infections in hip and knee prostheses were regressed on time of infection, systemic risk factors, local risk factors and the difficulty in treating the causing pathogen. The main outcome variable was “definitely free of infection” after two years as published. Age, gender, and BMI were included as covariables and analyzed in a logistic regression model. 66 male and 54 female patients, with a mean age at surgery of 68.3 years ± 12.0 and a mean BMI of 26.05 ± 6.21 were included in our survey and followed for 29.0 ± 11.3 months. We found a significant association (p < 0.001) between our score and the outcome parameters evaluated. Age, gender and BMI did not show a significant association with the outcome. These results show that our score is an independent and reliable predictor for the cure rate in prosthetic joint infections in hip and knee prostheses treated within a two-stage exchange protocol. Our score illustrates, that there is a statistically significant, sizable decrease in cure rate with an increase in score. In patients with prosthetic joint infections the validation of a risk score may help to identify patients with local and systemic risk factors or with infectious organisms identified as “difficult to treat” prior to the treatment or the decision about the treatment concept. Surgical management of this problem must be tailored to each patient and a thorough clinical and radiological assessment of the anatomical alignment should be carried out pre-operatively. The aim of this study is to assess the role of medial patellofemoral ligament reconstruction combined with tibial tuberosity transfer in patients with increased tibial tuberosity to trochlear groove (TT-TG) distance. Twenty-four patients (27 knees) over 2-years were operated on by a single surgeon, with standardised post-operative rehabilitation and follow up. Mean follow up was 31-months. Two patients had problems with recurrent instability, 1 had a traumatic re-dislocation at 2 years and a total of 4 required further operation for complications. Mean post-operative Kujala scores were 87.4 (SD 9.8). Combined medial patellofemoral ligament reconstruction and tibial tuberosity transfer is an appropriate treatment for patients with increased TT-TG distance. Preoperative MRI images of 16 Scheuermann’s kyphosis patients were analysed and compared to 16 patients with normal MRI images (control group). The CSAs were measured at L3-4 and L4-5. Both groups showed similar demographics and patient characteristics. The multifidus muscles CSA were found to be significantly smaller at L3/4 level in Scheuermann’s kyphosis patients (p = 0.022 on the left and p = 0.016 on the right side compared to control group). There was no significant change in multifidus CSA found at L4/5. The mean CSA of the extensor spinae muscles group were significantly smaller at all levels in Scheuermann’s kyphosis patients : p = 0.001 bilaterally at L3/4 and p = 0.015 right side and p = 0.009 left side at L4/5 level. This study shows that patients with Scheuermann’s kyphosis deformity have significantly smaller CSA of lumbar multifidus and extensor spinae muscles. Introduction This study examines patients’ perceptions of the usefulness of occupational therapy and assistive devices provided, in achieving discharge criteria and assisting with activities of daily living once home, following an Enhanced Recovery After Surgery protocol for total hip and total knee replacement patients.
Methods Two weeks following discharge, 197 patients who had undergone total hip replacement or total knee replacement were telephoned to ask how useful they found occupational therapy education and advice and the assistive devices provided.
Results 114 patients (58%) answered the call. Over 96% of patients found occupational therapy education and advice useful. Ninety-seven percent of posterior total hip replacement patients were still using the assistive devices 2 weeks following discharge, compared to 90% of anterior total hip replacement patients and 87% of total knee replacement patients. Over 95% of all patients found that assistive devices enhanced their ability to undertake activities of daily living, and were extremely or partly useful in achieving early discharge.
Conclusion Over 85% of patients who responded perceived the occupational therapist’s advice and education, and assistive devices provided, to be useful in achieving discharge criteria, and in enhancing their ability to undertake activities of daily living once back in a home environment.Permalink : ./index.php?lvl=notice_display&id=46024
